General Inclusion Criteria:
- 65 years of age and older
- 10 or more medications
- Appropriate for a group visit
Ideal Patient:
- Many recent medication changes
- Co-managed by private providers
- Patient unsure about specifics of medication regimen
- Provider suspects non-adherence
The patients are referred through an EHR consult put in by the primary care provider or specialty service. The patients receive a telephone call outlining the details of the clinic and invitation to participate. The patients are asked to bring all of their medications to the visit (prescriptions, over the counter medications, herbal and dietary supplements). The spouse and/or caregiver is also invited.
